  • PATIENT FORMS & PREP

For new patients, we need to get to know you a little bit before your initial consultation.

One of our Patient Coordinators (like Kelsey here) will send over a few forms for you to review carefully, sign, and return to us.

We’re a paperless practice, so be ready to receive your patient forms via text through our HIPAA Compliant Texting Service: KLARA®. We require all paperwork to be submitted before you enter the practice to insure a streamlined and optimized appointment.

We ask that you arrive 5 MINUTES EARLY to your appointment. This will streamline the time you have with your provider, ensuring you receive the attention and treatment you desire.

    AFTER YOUR TREATMENT

After your Injectable Treatment, Laser, or Microneedling session, you’ll ice to calm the treatment areas, then we dim the lights so you can recline and relax before checking out.

If you are taking home skincare products, we’ll walk you through the regimen to amplify and lengthen your treatment results.

We run transactions through a touchless process and email your receipt to you immediately.

Our team will text you via KLARA® the day after your appointment to check in on you. If you have further questions about post-care regimens, you have full access to the team here. If you've come in for an Injectable Treatment and have any bruising from treatment, text us on KLARA® and we'll book you for a complimentary laser treatment to zap it away.

Our team is in the office Monday through Friday and we're on KLARA® to chat with you directly.

IMPORTANT NOTES

In response to COVID-19, every patient receives our Patient Protocol Agreement via KLARA®. Patients must carefully review, sign, and send back this Agreement before entering the office for your appointment. This document carefully outlines the protocols we've put in place to keep you, our team, and our entire patient family at Center safe and secure.

Our Cancellation Policy is strictly enforced. Should you need to cancel or reschedule, you must let us know at least 24 hours (or 1 business day) in advance. If you do breach the cancelation policy, your card on file will be automatically charged $300 for any appointments 30 minutes and under. This is valid for both Virtual and In-Office Appointments, as well as any Follow-Up Appointments. For more information, please visit our POLICIES page.

We appreciate your interest in Center and know that the process of finding and trusting a qualified provider can be a challenging process. We’re happy you found us and we can't wait to meet you.
